1) Download the following
forms:
2) Complete all three forms — be sure to sign where indicated! — and mail all three forms along with tuition payment to:
3) Download the Teacher Recommendation Form. Print it out and give it to one of your child's teachers. Please have that teacher complete the form, then have him/her send it directly to Dancing Coyote Camp. 4) The Camp will evaluate your application, teacher's endorsement, and medical records. If it is determined that Dancing Coyote Camp can meet the needs of the student camper, and space is still available, the session requested is reserved. If the session you requested is unavailable, we will contact you by phone or e-mail to see if a different session will still work for you and your child. 5) You will then receive confirmation and further information from Dancing Coyote Camp by mail. 6) Any remaining tuition balance is due no later than 1 month prior to session start date. You may make payments with personal or cashier's check or money order. 7) Parents, download
additional information. These are some of the same forms that will be
mailed to you in the final information packet: 8) Be sure your student camper arrives at our designated assembly spot no later than 2:00 pm on Sunday, the first day of each camp session.
